Alzheimer's Day Care Resource Center (ADCRC)

Senior Working With Child

ADCRC's are established under the guidelines of the Older Californians Act to provide a program of specialized day care for participants with Alzheimer's disease or other dementia-related disorders whose care needs and behavioral problems may make it difficult to participate in existing care programs

Brown Bag Program

Programs are provided under the guidelines of the Older Californian's Act to distribute fruits, vegetables, and other food products to individuals 60 years of age and older living on low incomes as per 125% of the Federal Poverty Guidelines.

Congregate Meals

Programs in this area are provided under the guidelines of Title III C-1 of the Older Americans Act and are defined as meals served in a congregate setting which includes a meal providing one-third (1/3) of the recommended dietary allowance (RDA), and that complies with the Dietary Guidelines for Americans, provides nutrition and health promotion education and health promotion programs, nutrition risk screening, and opportunities for socialization.

Elder Abuse Prevention

Programs in this area are provided under the guidelines of Title VIIB of the Older Americans Act and are designed to develop, strengthen, and carry out the prevention and treatment of elder abuse, neglect and exploitation.

Family Caregiver Support

Programs in this area are provided under the guidelines of Title IIIE of the Older Americans Act and may include information to caregivers about available services; assistance to caregivers in gaining access to the services; individual counseling, organization of support groups, and caregiver training to caregivers to assist the caregivers in making decisions and solving problems relating to their caregiving roles; respite care to enable caregivers to be temporarily relieved from their caregiving responsibilities; and supplemental services, on a limited basis, to complement the care provided by caregivers.

Health Insurance Counciling and Advocacy

HICAP services are established under the guidelines of the Older Californians Act to provide counseling, advocacy, education and legal referral for Medicare Beneficiaries, including Medicare Beneficiaries by virtue of a disability, and those persons imminent of Medicare eligibility.

Programs are provided under the guidelines of Title III D of the Older American's Act and are designed to educate older adults to lead healthy lifestyles by reducing the risk of falls.

Home Delivered Meals

Programs in this area are provided under the guidelines of Title III C-2 of the Older Americans Act and are defined as meals provided to homebound older individuals providing one-third (1/3) of the recommended dietary allowance (RDA), and that complies with the Dietary Guidelines for Americans, nutrition and health promotion and nutrition risk screening.

Information and Assistance

Programs are provided under the guidelines of Title III B of the Older American's Act and are designed to link persons aged 60 and above, who are in need of assistance, to the appropriate programs and services in their communities.  This program is considered the entry point/introduction to the services system network.

Linkages Program

Linkages services are established under the guidelines of the Older Californians Act to provide a means of directly buying or paying for services or equipment for the purposes of stabilizing a client with emotional or cognitive impairments. To be eligible to receive these services, the client must meet one or more of the following criteria: impairment in one or more Activities of Daily Living (ADL); impairment in two or more Instrumental Activities of Daily Living (IADL); unable to manage due to emotional or cognitive impairment; or impaired due to a significant event or circumstance that has occurred in the past 12 months.

The intent of the program is to prevent the premature or inappropriate institutionalization of frail, at risk elderly and functionally impaired adults, aged 18 and older by providing case management as well as comprehensive information and assistance services.

Long-Term Care (Ombudsman)

Programs in this area are provided under the guidelines of Title VII A of the Older Americans Act and provides for advocacy services to or on behalf of residents of long-term care facilities, including, but not limited to: investigation and resolution of complaints; training of personnel; promotion of and attendance at resident and family councils; representation of residents' interests before governmental agencies; analysis and monitoring of the development and implementation of federal, state, and local laws, regulations, and other governmental policies and actions.

Respite Care

Respite Purchase of Service (RPOS) is established under the guidelines of the Older Californians Act to provide a means of directly buying or paying for respite care services for caregivers with primary responsibility for the care of frail elderly adults or adults with functional impairments. Respite care is defined as the temporary or periodic services that allow caregivers a break from their caregiving responsibilities.

Senior Friends

Senior Companion Program

Services were established under the guidelines of the Older Californian's Act to serve frail older adults in need of extra assistance to live independently in their own homes or communities.  Companions assist with activities such as simple chores, grocery shopping, and meal preparation as well as offer friendship to those who are isolated.

Legal Services were established under the guidelines of Title III B of the Older American's Act.  The purpose of the Legal Services Program is to increase availability and quality of legal services to individuals 60 years of age and older, and assist these persons with a variety of legal issues.
